At Casa de la Luz Hospice, we are more than an organization of hospice professionals – we are proud members of communities across southern Arizona. Compassionate, quality service is our mission. And to us, service means getting to know our patients on a personal level.

We know the people we serve, understand their needs, and tailor healthcare to ensure the best journey possible.

We’re passionately committed to making healthcare better every day by leading the industry in clinical excellence and service quality.

We empower our clinicians and staff to embrace the communities they serve, because the best care happens in the context of strong relationships.

Each patient and family we are privileged to serve is unique. Our ongoing mission is to design a specific plan to help you or your loved one live as fully and comfortably as possible in the final stages of life – in the comfort of your own home or place of residence.

Under the direct supervision of your physician, our highly skilled hospice professionals deliver quality, compassionate care. We also include family in the process, offering valuable information, guidance and – most of all – support.

Casa de la Luz Foundation was born in 2000 to transform the end-of-life experience into the final act of living well for those on hospice care in our community. For the past 21 years, Casa de la Luz has provided financial support to patients and families, as well as education and research about end-of-life issues.

June 2021 marks a huge transition for them as they rebrand to become Roots & Roads Community Hospice Foundation.  This rebrand will allow them to reach more hospice patients in need by expanding their focus to a larger area of Southeastern Arizona (Pima, Pinal, Cochise and Santa Cruz counties).

      Highlights from 2020 include:

      • 98 hospice patients and their families received more than $70,000 worth of financial support for things like housing, transportation and final arrangements
      • 97 people participated in bereavement groups and received Foundation-sponsored grief workbooks
      • 235 individuals attended their annual End-of-Life Conference, a unique, day-long educational event
      • 108 Veterans were honored for their servicethrough Foundation-sponsored ceremonies.

As the elderly population doubles over the next decade, and 65% of those at end of life choose hospice service, Casa de la Luz’s support of patients and families will be even more critical. They
